Oceanside Surf Institute does sell out and we run on a first come, first serve basis. Although we will make our best effort to try to accommodate everyone, there is only a limited amount of equipment.

Summer 2008 Registration Form Adobe Acrobat Download

You have the option of printing the form and filling it out by hand or you may use Adobe Acrobat to fill in the fields. To do so, click a field and type in the appropriate data and then print the file.
Don't forget! You will still have to manually sign the form!
